- In 2002, a vehicle collided with the platform on the north side. The vessel went underneath the platform and then collided with a well conductor bell guide. The vessel was a commercial fishing vessel.
- In 1995, a section of chiksan from a blowdown tank being rerouted to a high-pressure pump, The piping was in the process of being connected to the bump when the SSV on the well opened. The release of pressure caused the joint to strike workers who were working on rerouting piping. The workers had to be transported to a nearby medical facility.
